Chris Demko, MHA, FACHE

Chris Demko is the Operations Manager for the Northwest Ohio Pathways HUB at the Hospital Council of Northwest Ohio, which works closely with community agencies, as well as the PATH Project. Community Health Workers and other care coordinators are employed to connect clients to needed medical and social services.
